Aging Transition Advisor

Clear guidance for aging parent decisions.

I help Cleveland metro area, east side and west side families navigate home care, assisted living, and memory care decisions with honesty, compassion, and a focus on preserving independence whenever possible.

My philosophy

Independence when possible. Thoughtful placement when needed. Advocacy always.

My goal is not to push a move. My goal is to help your family understand the right next step. If home can still work safely with the right support, we will explore that. If assisted living or memory care becomes the best option, I will help you make that transition with clarity and care.

Who I help

You do not have to figure this out alone.

When caring for an aging parent becomes overwhelming, family roles start to distort. Adult children become case managers. Spouses become exhausted caregivers. Siblings become critics or avoiders. My work is to help you find the right support so your loved one is safe and cared for, and so you can be family again.

  • The adult daughter who has quietly become the family care coordinator.
  • The spouse who is exhausted and unsure how much longer they can keep going.
  • Siblings who disagree about whether it is time for more help.
  • Families facing memory loss, dementia, or a parent who refuses help.
Two ways to work together

Guidance that meets you where you are.

Aging Transition Consultation

For families who are unsure what kind of support is needed. We look at the full picture: safety, memory, care needs, caregiver burnout, family dynamics, budget, and what is realistic at home. Then I give you a clear next-step plan.

Assisted Living & Memory Care Placement Support

For families ready to explore assisted living or memory care. I help narrow options, coordinate tours, compare communities, and support the transition. This service is typically complimentary to your family, paid by the community if you move into one of my partner communities.

Why families call

The questions families bring to me.

Is my parent still safe at home?

We look honestly at safety, falls, medication, nutrition, hygiene, and isolation before deciding anything.

Is home care enough, or is it time for more?

Sometimes the right answer is more support at home. Sometimes it is assisted living or memory care. We sort through it together.

What if my family does not agree?

I help families talk through conflict, denial, guilt, and grief so you can make a decision everyone can live with.

Are we waiting too long, or overreacting?

I help you see the full picture clearly, so the next step is grounded in reality rather than fear or wishful thinking.

What makes this different

More than a list of communities.

Placement is not the first answer.

I do not believe assisted living should be the automatic answer. I believe in looking honestly at the situation and preserving independence whenever possible.

Family dynamics are part of the work.

Some families are close and loving. Some are complicated. Most are somewhere in between. I help families make practical, compassionate decisions without judgment.

Support does not end at move-in.

A successful transition is about helping your loved one settle, connect, and feel supported, and helping your family return to being family again.

About Jocelynn

An advisor in your corner.

I believe older adults deserve to remain independent for as long as they safely and realistically can. I also believe families deserve support before caregiving turns into crisis, resentment, or collapse. I am not here to push placement. I am here to help your family find the right level of support.

Let us look at what is really happening.

Tell me what is going on with your loved one. Together we will figure out the next right step, whether that means more support at home, assisted living, or memory care.
